Do you accept insurance?
We accept the following insurances:
- Kaiser Permanente (and Kaiser Medicare)
- PacificSource (not PacificSource Medicare)
- Legacy Health Plan
- CareOregon (OHP)
- Trillium (OHP)
Please note, OHP must be your primary insurance to be billed.
Out of Network Benefits:
We do not handle out-of-network reimbursement or submit claims through Mentaya. Instead, we provide a superbill that you can submit directly to your insurance provider for potential reimbursement.
